Effective health care depends on trust between patients and caregivers. The Toronto Palliative Care Associates, the physicians group that works out of the Temmy Latner Centre for Palliative Care at Mount Sinai Hospital is responsible to ensure the privacy and security of your personal health information.
You can trust us to respect your right to:
Access your personal health information
If, while under our care, you would like to access your health record, please ask your care provider to arrange a time to review your entire record with you.
If you would like a copy of the record, please contact our main number at 416.586.4800 x7884. The cost is $30 for the first 20 pages, and $0.25 for each additional page after that.
Request correction of incomplete or inaccurate information contained in your health record
If you believe that the personal health information we have recorded about you is inaccurate or incomplete, please discuss this with your care provider or contact our Privacy Officer and Managing Associate, Dr. Evan Schneider.
Have your personal health information kept confidential
We take steps to protect your personal health information from theft, loss and unauthorized access, copying, modification, use, disclosure and disposal. We conduct audits and complete investigations to monitor and manage our privacy compliance. We take steps to ensure that everyone who performs services for us protects your privacy and uses your personal health information only for the purposes of your care.
What is personal health information?
Personal Health Information is any information about you related to:
- your physical or mental health
- providing health care to you
- your health card number
- your substitute decision maker
- other information included in your health record.
Our information practices
We collect your personal health information directly from you or from the person acting on your behalf. The personal health information that we collect may include, for example, your contact information, medical history and records of the care you received by others care providers. We may also collect personal health information about you from other sources if we have obtained your consent to do so or if the law permits.
Uses and disclosures of personal health information
We use and disclose your personal health information as necessary to:
- treat and care for you
- receive payment for your treatment and care from OHIP
- plan, administer and manage the Centre and its programs
- conduct risk management and quality improvement activities
- conduct evaluation and research initiatives, approved by Research Ethics Board
- teach medical and other health-care students and professionals
- compile statistics
- comply with legal and regulatory requirements
- fulfill other purposes as required by law.
We may also include your mailing address in our contact list so that we can inform you about our other services and programs.
